Oklahoma State Department of Health Mobile Unit Will Offer Free Sports Physicals

Oklahoma State Department of Health Mobile Unit will offer FREE SPORTS PHYSICALS from 8:30 to 11 a.m. and 1 to 3 p.m. at Robson Fieldhouse on July 9.
A parent or guardian must provide consent for students under 18.
New Physical Forms will be available on site or can also be printed and pre-filled from the OSSAA website.
Parents DO NOT need to attend with their student if they sign the form ahead of time.
